Jefferson County Reentry Information

Jefferson County Community Corrections recognizes the importance of Reentry back into the Community after incarceration by an offender. We emphasize a community partnership approach in order to better prepare the offender’s transition. The process of transition begins prior to an inmate’s release and involves a collaborative effort by field services (Parole and Probation) and The Department of Corrections as well as many other partners and support groups targeted specifically for the offenders needs. Jefferson County Community Corrections will contact each inmate approximately 30-90 days prior to their scheduled release to discuss topics such as housing, employment/education, treatment needs, medication needs, transportation issues, support groups and most importantly measure what level of motivation they have in regards to changing their previous behaviors and belief systems. Evidence Based Practices show that targeting these topics prior to being released and following up on them will provide a higher level of success among this population.

Jefferson County Adult Community Corrections mission is to promote public safety and a healthy community by holding offenders accountable to the community and their victims, develop and access evidence based resources to help offender gain skills and develop competencies to decrease future criminal behavior and to provide intervention and prevention services before serious problems emerge.

HEALTH CARE

Health Care Safety Net Clinics are community-based providers who offer health services to low-income people, including insurance. Most safety net patients are OHP enrollees, the uninsured, and other vulnerable Oregonians who pay a sliding fee for primary care services. Primary care services include, but are not limited to:

  • Urgent care

  • Services based on local community need (mental health, dental, and vision)
  • Preventive care
